Meanwhile, the condition of a young patient whose diagnosis remains elusive continues to worsen despite all interventions, and İnan's instincts—now that he can no longer officially practice medicine—give the case an entirely new direction. However, the cost of this move will be paid by an unexpected person, leaving Elif right in the middle of a grueling ordeal where she will question both her professional future and the trust she has placed in İnan.

The memory of a brief moment that left everyone in suspense, İnan begins his first day as a doctor in his new life. Elif, on the other hand, has a new decision regarding İnan, and with this decision, she enters a serious change. Meanwhile, a patient from Toprak's past causes him difficult moments. While Toprak struggles to make a diagnosis without being able to communicate with his patient, İnan will once again clash with him and his decisions.

But she is also determined to teach Toprak a lesson.
