Jeff lost 200 pounds on his own in only one year with a rigid diet and exercise regimen. Jeff had facial surgery and underwent a series of rigorous medical tests in order to be considered for an extended body tuck, a medical procedure that would remove the large amounts of excess skin remaining on his frame. Jeff suffers complications during tests which puts his extreme makeover in jeopardy, and makes him question his decision to go through with the process. The long distance and time away from home and from his wife take their toll on Jeff, driving him to the very edge of self-doubt. Because of a prescribed high protein diet to fight anemia, he suffers another setback when he gains 15 pounds.
